We are not accredited school we train conducting professional seminars and the employer determines suitability for their purposes or supporting their in-house or contracted out safety training programs.

Our Policy on online
training is that if you do live training you have a better chance to live
safely.

ONLINE TRAINING; LIVE TRAINING AND EMPLOYER HAZWOPER TRAINING IS REQUIRED FOR
ANY POTENTIAL HAZARDS THAT COULD RESULT IN SERIOUS INJURY OR DEATH FROM ANTHING
THAT ARE BIOLOGICAL, CHEMICAL, FIRE OR EXPLOSIONS DANGERS; FAILURE TO FOLLOW
THOSE RULES OR YOUR EMPLOYERS SAFETY PROGRAM COULD RESULT IN DEATH OR SERIOUS
INJURY. Therefore, online training is supplemental to on-going
training with your employer or during the course of your profession and work.
However, we will train online you agree and have been warned that a competent
Industrial Hygienist or other technically qualified person assists you in the
total endeavor, so you know how to select and use the safety equipment, to meet
the OSHA requirements and get Site Specific Safety training for the hazards at
the workplace.

approved for or our associates e.g. OSHA DOL Ten Hour and Thirty hour
construction programs that we get cards from OSHA's Official DOL Training
Program and follow the latest program
requirements for such courses minimum of three people are required for
construction 10 and 30 hour courses approved by the DOL OSHA. (We are NOT under
any agency or affiliated with the federal government and disclaim that as we are
not affiliated with OSHA and we have no conflicts with the federal government.)
Any materials from the federal government we use you agree to be bound to those
federal agencies terms and disclaimers and including ours.
Other Terms and conditions: SERVICES REQUIRE A SIGNED AGREEMENT and/or REGISTRATION FORM, Valid ID required and/or credit card. By registering for training, Purchase Order, other and if paid by credit card you are agreeing to the credit card company’s terms and conditions for payment and the following terms: Contract for payment on demand unless indicated otherwise. Prepay at time of service unless credit application on file or as an established customer. All courses are tentative until invoiced and payment is received and trainees sign in. Client agrees to pay o n demand unless exception by credit application within Thirty Days (30 Days) unless established account that allows Net 60, past due an interest fee of 15 % Per Mo. or the highest allowed by the state the lower of the two shall apply, will be charged thereafter; non-payment subject to costs for recovery. We reserve the right to Sub-Contract all or a portion of our services. Other binding terms shall be on registration signed for payment these terms and those are binding and the latter shall supersede in the event of any conflicting terms.
[Client and all related persons receiving service and / or by the authorized person signing agreeing to Hendy; Hendy Company, Super Safety or Chicago Safety Institute, premises, or any others e.g. staff, instructors & our contractors, and any property owners collectively (us) for any of our services agree to hold us harmless, indemnify, and defend David for any accidents, personal Injury, lost time, injuries, property damage and/or to be not liable for any circumstances.] Furthermore, you shall pay any attorney fees for us to recover our loss.] Other than extremely gross negligence. Cancellation Policy: To Cancel FAX and Call ten (10) business days before the class to be free thereafter no refunds, (800) 275-8239 and FAX (773) 538-8080 While we do our best to accommodate your schedule we sometimes have limited seats, supply and demand.
You agree [CANCELLATION less than < 10 DAYS WE CHARGE FULL PRICE FOR SAFETY TRAINING / EXCEPT FOR MSHA less than < 3 DAYS and OSHA 10 or 30 Hour DOL Courses. Payment on demand upon invoice or for online or correspondence at upon invoice and/or registration. No exceptions without our written approval for cancellation (books and the original certifications must be recovered). [NO CANCELATIONS BETWEEN December 15 and January 1, WEEKENDS OR HOLIDAYS will not be refunded] No refunds in order to cover our cost, no show no refund, and online training or correspondence no refunds after 24 hours of registration. If WE CANCEL there is a FULL REFUND for cancellation. We reserve the right to cancel, or refuse admittance of anyone for any reason, or due to availability of instructors, assess & Access locations or training needs or based on supply and demands to modify prices at any time. We may cancel e.g. due to scheduling & to answer our client’s and Emergency Response needs. You agree your Cancellations and rescheduling fees shall apply $50.00 minimum in addition to our written approval if full price is waived; (Example with letter for dated after booking drug test failure & military activation excluded, Dr. Letter Medical Emergency, Court letter Jury Duty). We do not refund last minute project cancellations as we have costs too. We can perform at your facility on a cost plus 15% basis, $18.50 per hour for travel hours more than 40 miles. Seats guaranteed only upon payment. Pay by Corporate, or Certified Check, Money Order or Credit Card w/ processing fee. Late payments ANY course incur interest 15 % Per Month (or maximum allowable by law (No Exceptions) and [VOID ALL DISCOUNTS and for HAZWOPER Effective March 20, 2017, If you don’t have an approved credit account here please apply, or pay prior to services. Paying after 4 hours day of HAZWOPER training is late registration and list price $995.00 for 40-Hour Courses and pay $250 for 8 Hour Refresher except correspondence that is thirty days from the date of registration where applicable. Anyone may cancel in the first 4 hours (four) for a refund and EXCEPT less when Company has guest paid speakers or instructors travel and shipping we incur cost plus +15%. We are not responsible for any unforeseeable travel delays we cannot anticipate or delivery dates. If at our location anyone may cancel for free if signed in on time and within first four 4 Hours only; drop outs and no shows shall be paid for taking up valued limited seating, exceptions are Medical Emergency, Military Call Out and or Jury Duty.
If any part of this agreement shall be found not applicable it shall not restrict the other parts from being applicable all participants shall be covered by their own or employer(s)’ worker's compensation, or their own insurance or employers and assume their own responsibility for any whatsoever accident or causation. That is solely the individual or employer(s)' responsibility you agree your employees are physically fit to get this training 29 CFR 1910.120(f). Client, trainees, and all related parties using our services agree to

Responsibilities Disclaimer
[Do not leave personal belongings inside the facility unattended. We are not responsible for any lost, stolen, or misplaced items at our premises, training center, or any outside conference room.] We do take a reasonable efforts to protect our clients. The premises utilizes video surveillance and may or may not be operational at any time. Any activities on or phone lines or in and around the perimeter of our facility may be recorded for training, quality or security purposes.
At no case shall liability exceed the amount contracted for. This is because we do not supervise your employees. Any discrepancies, ambiguities and conflict with other parts of this website the more stringent shall apply and is subject to the interpretation of the this companies senior official. We try to always serve on a first come first serve basis, please register in advance.
OTHER TERMS
Effective June 29, 2017 Mandatory Late Policy:
Trainees must attend all scheduled hours [No refunds for missed courses hours] all make up work shall be disapproved or approved by instructors thus made up as determined by instructor or prearranged, severe tardiness reserve the right to be paid by company or person getting our services, $50.00 per hour or $400.00 per 8 hour day. We may arrange time at our Instructor’s convenience, since in some cases we may not be able to make up training the trainee may have no option other than retaking the course if available, they will have to pay for the course again if this is done. For missing OSHA classes and doing make-ups we reserve the right to double the charge as we have to meet OSHA's reporting requirements and our workload increases from such events.
OSHA Authorized courses will be subject to OSHA's latest Program Requirements, such as submitting why a participant missed the class or make up rules OSHA has and we reserve the right to charge double for such circumstances. Where you bring in an interpreter you agree the course time shall be doubles and the cost doubled, as OSHA requires this to facilitate time for translations.
No bad disruptive attitudes (verbal warning may be given), anyone wearing gang (such as those listed on the www.fbi.gov colors symbols that insight fear or intimidation not permitted on the basis FBI's national gang threat ASSESSMENT 2009 or current rules/standards, weapons, reasonable suspicion of Drugs or Alcohol use during training, sexual harassment will not be tolerated in training and will be grounds for dismissal from training. In rare case we might with long standing clients provide an explanation why we were having trouble servicing their account however, this is not always done. No throwing anything while in training is allowed or disruptive behavior sexual advances, abusive language or any other form of harassment to others. Any such incident must be reported immediately to the Instructor within the same day company shall not be responsible or liable for trainees from the same company as they should deal with such matters through there own regular channels and report the matter when it occurs to us. Differences of opinion are tolerated unless it becomes disruptive (taking time from time allocated for training).
Instructors reserve the right to reschedule trainee for cause missing training hours late or disturbances, dismiss; Trainee and/or Trainees Company shall forfeit pay to us for original training and pay again for rescheduling.
